  • Demonstrates a basic knowledge of and endorses the CBHI values: Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative and Integrated, and Continuous Quality Improvement.
  • Maintains a mixed caseload of TM, IHT and ICC youth
  • Conducts comprehensive home-based clinical assessments including the CANS; risk management, safety, and treatment/care planning
  • Works with each youth/family to assemble a Care Planning Team composed of family members, their natural supports, and appropriate collaterals
  • Coordinates and facilitates regular Care Planning Team meetings to develop an individualized Care Plan for each enrolled youth/family
  • Works closely with the FCT Family Partner, the family, and the Care Planning Team to implement, monitor and modify the Care Plan as needed, and serves as an advocate/liaison for the family when appropriate to access new or different services.
  • Collaborates and communicates regularly with other key service/clinical providers
  • Effectively communicates and builds strong rapport with youth, families, colleagues, and/or community members in a manner that is sensitive to their racial/ethnic/cultural/class backgrounds.
  • Maintain client records based on role, including completing comprehensive assessments, CANS, care plans, treatment plans, intervention plans, safety plans, contact notes, and discharge summaries.
  • Maintains program productivity standards
  • Works with insurance companies to obtain service authorizations
  • Participates in all required meetings, trainings, supervisions, quality management, and related activities as may be directed by FCT program management.
  • Follows all program and CHL policies and procedures.
  • Maintains the highest standards of ethics and demonstrates professional attitude and conduct
  • Works assigned hours, which includes some weekday evening hours (up to 8pm)
  • Travel is required as part of this position, which includes travel to/from families' homes, community settings, and CHL sites.
  • Participates in on-call rotation that includes providing telephonic support to enrolled families
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications:

  • Grade C06: Clinician II- Intensive Care Coordinator: requires minimum qualifications of Bachelor's degree in psychology, social work, or related human services field
  • Grade C12: Clinician III: Intensive Care Coordinator:Master's degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirementsfor independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts. Requires licensure as LMHC, LCSW, LMFT, or is actively working towards licensure in the Commonwealth of Massachusetts.
  • Minimum of 1-year clinical experience with children, adolescents and families with behavioral, social and/or emotional difficulties.
  • Proven ability to effectively engage youth and/or families, successfully advocate for clients and to collaborate well with other professionals
  • Comfortable providing home-based and community-based services.
  • Demonstrate strong work ethic, ability to work independently and ability to contribute to team objectives
  • Demonstrate effective communication, writing, and organizational skills.
  • Ability to use a computer-based information system.
  • Current valid US-issued driver's license and ability to provide registered and insured automobile for work-related purposes
